Outcomes

Measurable results under risk and value-based contracts.

Ascend Rx is built to move the metrics our provider and plan partners are accountable for — adherence, utilization, and total cost of care.

Real impact on value-based metrics

What changes when pharmacy is part of the care model

Coordinated pharmacy care moves the measures that determine quality performance and shared savings. Below is the impact we typically see with value-based provider partners.

MetricTypical impactWhy it matters
Medication adherence ≥94%, a +10% to +15% improvement Directly drives STAR Ratings, quality incentives, and bonus pools
STAR Ratings — medication measures Top 5% nationally on Proportion of Days Covered Tied directly to Medicare Advantage bonus payments and rebates
Hospitalizations & readmissions Up to 20% reduction in medication-related events Fewer adverse events from non-adherence and therapy duplication
Average prescriptions per member 8% reduction Lowers Part D cost and eliminates duplicative therapy
Pharmacy cost per member per month Materially lower Rx PMPM Improves total cost of care and medical loss ratio
Care team workflow Gaps identified earlier Real-time medication data makes outreach targeted instead of reactive

Figures reflect results observed across value-based provider partners. Actual results vary by population, contract structure, and baseline performance.

Example provider group results

Same patients, same market — different pharmacy

A South Florida Medicare Advantage provider group compared two groups of its own Medicare patients over a three-month period in 2024: those filling with Ascend Rx, and those using outside pharmacies. The patients served by Ascend Rx used substantially less acute care.

Metric Ascend Rx patients Outside pharmacy patients Difference
Hospital admissions per 1,000 47228↓ 79%
Emergency room visits per 1,000 140410↓ 66%
Readmission rate 10%28%↓ 18 pts
↓79% Fewer hospital admissions per 1,000 patients
↓66% Fewer emergency room visits per 1,000 patients
10% Readmission rate, against 28% for patients using outside pharmacies

About this analysis. This is an observational comparison of two groups of Medicare patients within a single provider group over a three-month period in 2024. Patients were not randomly assigned, and the groups were not matched for baseline health status, so the results show a strong association rather than proving causation. We are happy to walk partners through the methodology and the underlying data in detail.

Why it matters

The problem these numbers are solving

Published industry research. These are not Ascend Rx results — they describe the gap our model is built to close.
